Who we are : La Clinica de La Raza is a non-profit, community-based health center established in 1971 to address health barriers and create better lives for the underserved. We have proudly grown into 35 sites across Alameda, Contra Costa, and Solano County.

Job Summary:

Under general direction and supervision of the Clinical Health Education Manager, the individual in this position is responsible for providing clinical training and oversight to staff providing nutrition services across all medical sites in Alameda, Contra Costa, and Solano counties. This includes providing on-boarding, and ongoing training, observation, chart review, and coaching for dietitians and health educators, as well as supervision of dietetic interns. The individual works closely with the Clinical Health Education leadership team to develop monthly trainings and to engage in ongoing quality improvement efforts to support improved efficiencies, outcomes, and integration with the medical teams. The individual will provide direct nutrition services to clients at their home site.

Major Areas of Responsibility, include but are not limited to:

Supervisory Responsibilities:

  • Provides initial and ongoing training, consultation, and oversight in nutrition-based chronic disease prevention coaching to health educators and dietitians at La Clinica’s primary care sites.
  • Helps to screen, interview, and select candidates for nutrition-related positions as needed.

Duties/Responsibilities:

  • Observes dietitians and health educators in individual and group nutrition counseling and conducts chart audits; provides feedback to staff and supervisors for use in appraisals.
  • Helps to plan and conduct nutrition related group trainings at regular agency-wide Clinical Health Education meetings.
  • Develops and updates nutrition-related standardized electronic documentation notes.
  • Develops key messages, coaching tools, patient handouts related to chronic disease prevention and self-management; updates the staff Intranet and external website with these materials.
  • Engages in regular quality improvement meetings with the Clinical Health Education leadership team to make ongoing system improvements and plan upcoming trainings.
  • Facilitates periodic registered dietitian (RD) meetings and annual RD peer review.
  • Serves as preceptor to RD interns to assist in the career pipeline development of bilingual, bicultural dietitians.
  • Keeps informed of the current literature on evidence-based best practices in nutrition counseling and nutrition-related chronic disease prevention and management.
  • Reviews, updates, and approves nutrition-based chronic disease prevention curricula used by clinical health education staff to ensure it is based on the latest evidence and best practice.
  • Acts as consultant to agency regarding project development and grant submission related to nutrition-related chronic disease prevention and self-management.
  • Conducts nutrition screening, assessment and counseling, Medical Nutrition Therapy and implements nutrition care plans for patients with chronic disease (obesity, hypertension, hyperlipidemia, and diabetes) & prenatal patients.
  • Other duties as assigned by supervisor.

Minimum Job Requirements:

Knowledge, Skills, and Abilities:

  • Knowledge of contemporary principles and practices of nutrition, nutritional value of foods, interpretation and implementation of special diets, and meals planning and preparation that is culturally appropriate.
  • Ability to work across cultures and demonstrate support of diversity, equity, and inclusion.
  • Ability to foster a positive team of professionals and to foster a positive work environment.
  • Must have good organizational skills.
  • Ability to develop, implement, and evaluate nutrition education activities and coordinate them with other Departmental programs.
  • English/Spanish oral and written proficiency preferred.

Education and Experience:

  • Requires completion of Dietetic and Food administration Program from an accredited university, RD, or RD eligible, plus one year experience working in a community health care setting. Master’s in nutrition/Dietetics preferred but not required.
  • Must hold a current BLS (Basic Life Support) Certificate. You will have 30-day grace period to obtain your certificate from the date of hire.
  • Expertise in supervising, training, and evaluating staff. Prefer a minimum of 2 years of supervision.
  • Experience with Motivational Interviewing/Learner Center Interviewing preferred.

Physical Requirements:

  • Remaining in a stationary position, often standing, or sitting for prolonged periods.
  • Bending/lifting, pushing/pulling, adjusting, or moving objects weighing up to 15 pounds in all directions and occasionally more.
  • Repeating motions that may include the neck, wrists, hands and/or fingers.
  • Upper extremity demands: Reaching above shoulder and/or below waist.
